You are fully seen and tracked — this is the baseline detection state for most creatures in normal conditions. Observing requires a precise sense (usually sight).
Even an Observed creature can be Concealed (DC 5 flat check to target) if in mist or dim light. Observed only tracks whether a creature's location is known and it can be targeted; see Concealed, Hidden, Undetected, and Unnoticed for the full detection ladder.
