You are difficult to see due to fog, smoke, or other obscurement — but your location is still generally known. Attackers targeting you must succeed at a DC 5 flat check or their action has no effect.
Concealed doesn't change your detection category — a creature in light fog is still Observed. Compare with Hidden (location known, DC 11) and Invisible (automatically Undetected to sight-only creatures).
