Your location is known but you can't be directly perceived. You are Off-Guard to creatures you're hidden from. Creatures targeting you must succeed at a DC 11 flat check before rolling their effect; failure means the action is still expended but has no effect.
Contrast the detection states: Concealed (DC 5, location known, partially visible) → Hidden (DC 11, location known, not visible) → Undetected (DC 11, location unknown) → Unnoticed (presence unknown).
