The affected creature is completely unaware of your existence — not just your location, but your presence. An Undetected creature might also be Unnoticed. Abilities that require total surprise (sneak attack from true surprise, certain ambush effects) often require being Unnoticed.
Unnoticed is the strongest form of concealment in the detection ladder: Concealed → Hidden → Undetected → Unnoticed.
