Unconscious

You are asleep or have been knocked out. You can't act. You take a −4 status penalty to AC, Perception, and Reflex saves, and you have the Blinded and Off-Guard conditions. When knocked unconscious, you fall Prone and drop held items unless the effect specifies otherwise.

Waking up

  • At 0 HP (dying): you wake when healed to 1+ HP. See Dying.
  • At 0 HP (stable, not dying): you naturally return to 1 HP and wake after a period determined by the GM (minimum 10 minutes).
  • Above 0 HP: you wake when you take damage (if not dropped to 0), receive healing, are shaken awake (Interact action), or succeed at a Perception check against nearby noise (DC 5 for battle).

An Unconscious creature can still be perceived and targeted normally — it's just completely helpless. Attackers targeting an Unconscious creature don't need to deal with any detection conditions.
