The whirlpool
The lake has a throat, and it is always hungry for what swims near.
Near the dam at Three Coven Lake's east edge, a great drain draws water into the river Sruth. Get too close and the current catches you, dragging you closer until you can no longer swim free — down the funnel and into the pipe (1d10+2 damage with advantage, ignores armor). Roll +CON: on a 10+ you're spat out into the river, alive but beaten; on a 7-9, you're out but mark a debility and lose some gear; on a 6- you emerge the same, and also at Death's Door.
