Conditions Reference
Complete PF2e condition reference organized by type: attitude, death/dying, sensory, physical, and mental/status conditions with their mechanical effects.
PF2e conditions are grouped into categories. Many have numerical values (e.g., Frightened 2) that modify their severity.
Attitude Conditions (NPC disposition):
- Hostile: Will attack or hinder.
- Unfriendly: Actively opposes you.
- Indifferent: No particular feelings.
- Friendly: Likes you, willing to help within reason.
- Helpful: Goes out of their way to assist.
Death and Dying:
- Dying (1–4): Unconscious at 0 HP; die at Dying 4. Recovery Checks each turn.
- Doomed (1–3): Reduces maximum Dying value (Doomed 1 = die at Dying 3).
- Unconscious: Cannot act. Off-guard and blinded.
- Wounded (1–3): Added to Dying value on subsequent unconsciousness.
Perception and Senses:
- Blinded: Cannot see; all creatures are concealed (DC 5 flat check to target).
- Concealed: Target must flat check DC 5 to hit.
- Dazzled: All targets treated as concealed.
- Deafened: Fail auditory Perception checks.
- Hidden: Attacker must succeed on flat check DC 11 to target.
- Undetected: Attacker must guess your location.
Physical Conditions:
- Grabbed: Off-guard and immobilized. Manipulate actions require DC 5 flat check.
- Paralyzed: Off-guard, cannot take actions or reactions (except purely mental).
- Petrified: Turned to stone; cannot act.
- Prone: Off-guard; −2 circumstance to attacks. Can only Crawl or Stand. Take Cover for +4 vs ranged.
- Restrained: Off-guard and immobilized; can only Escape or Force Open.
- Slowed (1–3): Lose actions at start of turn equal to value.
- Stunned (value): Lose actions equal to stunned value (reduces as actions are lost). Overrides slowed.
Mental and Status:
- Clumsy (1–4): Status penalty to Dex rolls and AC.
- Confused: Attack random targets.
- Drained (1–4): Penalty to Con checks and max HP per level.
- Enfeebled (1–4): Penalty to Str rolls and melee damage.
- Fascinated: Cannot act toward other things.
- Fatigued: −1 to AC and saves.
- Off-Guard (formerly Flat-Footed): −2 circumstance penalty to AC. Does NOT remove Dex modifier. Does NOT prevent reactions — you can still use Attack of Opportunity, Shield Block, etc. while off-guard.
- Fleeing: Must move away from source.
- Frightened (1–4): Status penalty to all rolls; decreases 1 per turn.
- Quickened: Gain 1 extra action; limited uses.
- Sickened (1–4): Status penalty to all rolls and DCs.
