Sickened always includes a value. You take a status penalty equal to that value to all rolls and DCs. You can't willingly ingest anything. You can spend one action retching to attempt a Fortitude save against the effect's DC — success reduces Sickened by 1, critical success removes it entirely.
The Fortitude save DC to retch is set by the effect that caused Sickened. If no DC is specified, use 15. Sickened doesn't automatically decrease over time — retching or specific remedies are required.
