Drained always includes a value. You take a status penalty equal to the condition value to all Constitution-based rolls and DCs. Your maximum Hit Points is also reduced by your level × the Drained value.
The HP reduction from Drained is applied to your maximum — your current HP may exceed your new maximum immediately after being drained, in which case it is immediately reduced to match. Drained decreases by 1 each full night of rest.
